For Our Patients


The staff of Pediatrics Plus strives to complete all forms requested within a reasonable amount of time, 3-7 business days. If your child is planning on playing any spring/summer/fall sports, attending any camps and for those who are entering college, please plan accordingly so that we can get your form completed and returned to you before your deadlines. In the event, that we are asked to expedite a form (i.e. 24-hour turn around), please be aware that there will be a $10 fee charged. Please help us to provide you with the best service possible by getting your forms to us in a timely manner.

Our practice supports vaccinating all children according to the guidelines as outlined by the Academy of Pediatrics and the Center for Disease Control. Vaccines have been proven to be safe and effective in preventing diseases and health complications in children and adolescents. We are happy to answer any of your questions and provide you with information regarding the vaccines provided in our office. If you’ve decided that you do not wish to vaccinate your child(ren), we unfortunately will be unable to accept your child(ren) as patients.


Vaccine Information Statements (VISs) are produced by the Centers for Disease Control and Prevention (CDC). VISs explain both the benefits and risks of a vaccine to adult vaccine recipients and the parents or legal representatives of vaccines who are children and adolescents. Federal law requires that VISs be handed out whenever certain vaccinations are given (before each dose). For more information, please visit

Have questions and/or concerns about breastfeeding and newborn care? Come join us for "mommy and baby" support group to led by Pediatrics Plus clinician Marci Panapada, APRN. In addition to being a Certified Nurse Practitioner, Marci is also a Certified Lactation Counselor and a breastfeeding mom.
